The ETHS Sports Medicine Clinic
Evanston Township High School Athletic Training is proud to partner with the ETHS School Based Health Center & our team physicians to host a weekly, in-house clinic where student-athletes can be seen completely free of charge. No insurance billing, no out-of-pocket fee.
What is the Sports Medicine Clinic?
Our weekly Sports Medicine clinic is a block of time every Monday afternoon where our team physicians, Dr. Walton or Dr. Dunlap, evaluate in-season student-athletes in need of further care for their injury.

The clinic is held within the ETHS Health Center nearest the Main Office (H101). The clinic is offered every Monday (excluding holidays), as necessary, beginning at 2:30p.
Does my child need anything prior to the visit?
The only requirement that must be fulfilled prior to a visit is a NorthShore Consent for Health Services Form. This will allow our team physicians to see our patients. The form must be filled in its entirety. The form can be accessed here, however, the athletic trainers have physical copies in their office. All student-athletes must consult with the athletic trainers in order to be placed on the clinic schedule. This is to ensure the clinic time is reserved to those who need it most.

Additionally, any documentation from other providers regarding your child's injury can be given prior to the appointment to be copied and shared with our team physicians.

We do our best to schedule students according to injury severity/appointment status

(ex: Student A with an ACL rupture may be considered to go ahead of Student B with lower back pain. A follow-up appointment might go ahead of a new patient appointment, since follow-ups usually do not take much time.)

As our clinic is held in the ETHS Health Center, our team doctors do NOT have immediate access to any imaging equipment.

If as a result of a clinic evaluation your child requires imaging (x-ray, MRI, etc.) for their injury, ​the athletic trainers can assist in making an appointment with our team doctors at their offices.

While an initial evaluation, as well as any follow-up evaluations in the clinic are free of charge, any additional appointments made outside of the clinic will have to be made using insurance coverage.

While our team physicians at Endeavor accept most major insurance plans, there is a possibility that a specific insurance plan may not be accepted.

Any patient whose family is under a Medicaid or other federal insurance plan is welcome to be seen in the clinic.

However, as mentioned above, if your child requires a follow-up outside of the clinic then we will have to refer to providers that are under your plan.​

ETHS Athletic Training will assist you in finding providers; additional information about your insurance plan may be needed to do so.
